Case Management

The primary goal of case management is to ensure HIV positive people in Hawaii receive the support they need to live with and ultimately survive AIDS. Life Foundation clients are assigned to a case manager who provides the following services. All services are free and confidential.

Advocacy and Community: Life Foundation advocates for clients' rights, and helps clients learn to help themselves. Opportunities are provided to socialize and interact with other clients. Life Foundation also provides referrals to other programs when necessary.

Meal Program:
Life Foundation offers monthly emergency assistance of canned goods and toiletries from our food pantry. Lunch is provided on Wednesdays (12:30 - 1:30 p.m.) and dinner on Fridays (6:30 - 7:30 p.m.).

Medical Assistance:
Case managers link clients to health care and life-saving medications. Clients can consult with the staff nurse regarding their prescriptions and management of side effects. The RN will also provide on-site support and advocacy to clients who are hospitalized.

Med Buddies:
This program pairs mentors with buddies, providing support and skill building to clients who are struggling to adhere to their medication regime.

Financial Assistance: Case managers assist clients in applying for financial benefits that they may be eligible for, such as Food stamps, SSI, and SSDI.

Native Hawaiian Program: This program addresses the needs of HIV-positive native Hawaiians in our community and provides culturally sensitive services to individuals and families affected by HIV/AIDS. In addition to standard case management services, the native Hawaiian case managers provides clients with access to traditional native Hawaiian healing practices, including lomilomi and ho'oponopono.

Kukula Ola Program: This program expands existing HIV services to meet the needs of people living with both HIV and mental illness. These services include mental health case management, psychiatric care, psychotherapy, and emotional support from a certified peer specialist.

Counseling: Life Foundation offers personal counseling to clients who have specific issues or problems they want to address. Doctoral or masters level interns are available to provide individual sessions. Counseling sessions may take place at Life Foundation, or in other settings, like a home or prison, if need be.

Support Groups: Life Foundation offers various HIV-positive support groups for those seeking social interaction and emotional support in a safe environment. For a listing of dates and times of support groups, refer to 'Lifebeat,' our monthly client newsletter.

Prevention for Positives: This peer support program offers guidance and emotional support from HIV-positive individuals for HIV-positive individuals. The focus is on shared experiences, risk-taking behavior, and safer-sex practices.

Educational Seminars: Life Foundation offers educational talks and presentations from guest speakers. Topics include issues such as wasting, Social Security's Back to Work program, and hepatitis.

Positive Living for Us (PLUS): Developed by HIV-positive people for HIV-positive people, PLUS weekend outlines everything clients need to know to live long and well with HIV. Like a mini-conference, PLUS occurs annually and features guest speakers, educational lectures, and support groups.
