Becoming a Client

Anyone who is HIV-positive and lives on Oahu is eligible to become a Life Foundation client. The first step in becoming a Life Foundation client is to complete a pre-screening on the phone with a case manager. The pre-screening and subsequent face-to-face interview will give you an opportunity to learn more about Life Foundation's services, and give your case manager an opportunity to learn more about you and your experience of living with HIV.

To schedule a pre-screening, or to learn more about the services Life Foundation offers, call 521-AIDS. If you are unable to come to Life Foundation's office, one of our case managers can meet you at your home or anywhere in the community (e.g., hospital, prison, beach).
